To refer a case to us, please complete a Forensic Evaluation Request Form through the Physicians for Human Rights website:

Please select "Clinic Direct Referral."
Under "Clinic Information," select Philadelphia Human Rights Clinic. Please complete the subsequent form.
Please note: All in-person examinations must take place in or around Philadelphia.
